List of required documents
Below you will find a list of requirements you will need for employment:
- Valid HHA or PCA Certificate
- I-9 (US Passport, ID Card, Permanent Resident Card) S.S. Card Professional Reference Letters(Previous Employment Preferred) Physical Statement (must be dated within 6 months)
- 2 Negative PPDs OR 1 Negative TB Gold Test (must be within 1 year)
- Positive PPD (requires the following)
A) Written documentation by a doctor with a history of positive PPD
B) Negative Chest X-Ray Report - Rubella Immunity Proof (lab printed report)
- Rubeola (measles) Immunity Proof of (lab printed report)
- Flu Vaccine/COVID-19 (1st and 2 Dose)
- Drug Screen (must be dated within 6 months)
