Medical Social Work

Personal-Touch Home Care’s Medical Social Workers are a vital part of the care team. Our licensed Social Workers collaborate with you, your physicians, and your care team to help solve problems and care impediments and assist you in reaching your goals. Social workers perform a range of services that will include but are not limited to:

• Assessment of medical/nursing requirements as it relates to the home situation, financial resources, and the community resources available
• Intervention services to remove impediments to effective care/treatment
• Referrals to community resources
• Assisting patient, family, and team members with social, emotional, and environmental issues relating to the patient’s health problems
