Personal-Touch Home Care’s Home and Community Based Waiver Program helps eligible individuals receive support while remaining in the comfort of their homes and communities. Services include:

• Case Management: Assistance with coordinating services, developing individualized care plans, and addressing changing care needs.

• Attendant Care: Support with daily activities such as bathing, dressing, grooming, and mobility to promote comfort and independence.

• Skilled Respite: Temporary care provided by qualified nursing professionals for individuals with skilled care needs, giving regular caregivers time to rest.

• Non-Skilled Respite: Temporary assistance with personal care, daily routines, and supervision while regular caregivers take a break.

This program serves individuals who meet nursing home eligibility requirements and applicable waiver program criteria. Currently available only in Kentucky.
